Is it possible for throw out bearing to slip out of clutch fork?

The clutch in my parents 2005 PT Cruiser has almost stopped working. They say it happened pretty quick so I took a look at it. The clutch fork doesn't look straight to the clutch. It is leaning down which makes the rod on the slave cylinder push at a downward angle. It's really hard to see but seems like the throw out bearing has slipped out on one side, is that possible?

I doubt it "slipped out", but in may have failed and come apart! Most front wheel drives do not have an inspection cover, My PT is an auto. I believe it will have to come apart to verify. I have a second gen (2008) service manual shows a piece, that's under the trans, bolts to both block & trans. When comes off you might be able to see inside that area. HOWEVER, to fix it trans will have to come out anyway!! So @ 80,000 there is usually a lot of life left in them, Lots of then approached 150-200 k. If you have ever done ANY FWD clutch, there all about the same, time consuming be easy. Take a good look at the slave cylinder though before to make sure it's not that. Have fun!

You are correct, the throw out bearing was completely disintegrated. There was hardly anything left of it. My stepdad is really hard of hearing so if it made any sounds of impending doom he didn't hear it!
Replaced the clutch and throw out bearing and they work fine. Had a little trouble figuring out the slave cylinder and how to get it working again but it finally came around. It releases a little to close to the floor but it works fine.
